According to Moffat at The Doctor Who Festival in Sydney, he welcomes the recent real world developments in replacement organs, as long as they mean he can live to a billion years old...

i think that i was one of the only people there in normal clothes. You had a Brigadier, a couple of Tennants, the odd Tom Baker, several Osgoods and one fellow who was meant to be Capaldi but looked more like Jon Pertwee circa 1993.

Pete: Both Davo and C Baker's agents were a little stubborn when it came to the fee their clients would get for painted covers... why is why many of their novelisations don't feature a painted rendition of the Doctor on the cover.

Colin Baker is certainly noticeable in his absence on the covers of the Target novelizations. His first story, The Twin Dilemma, was crying out for a picture of this brash new Doctor in his garish, colourful costume. Instead, we got a rather boring, generic alien.

In many of the original publications, Patrick Troughton hardly appears on the covers either. Apparently, the reason for this was that for several years they only copied the likeness of the incumbent Doctor. Since the majority of his books were written in the 70s and early 80s when Pertwee and T Baker were at the helm, poor old Pat didn't qualify! I recently read the brilliant The Enemy of the World in which Troughton played both the Doctor and an evil dictator who facially resembled the Doctor. But the cover wasn't exactly inspiring.
